Home Help Services

Home Help Care is designed to give consistent support to individuals who are unable to care for themselves adequately and maintain daily activities. Better Quality of life for our clients is the goal. Advanced Health Group provides their clients the opportunity to hire people they trust such as friends or a family member to care for them or we carefully find a perfect match for you. This program is 100% covered by Medicaid and will pay for someone to assist qualifying clients with:

  • Preparing Meals and Cleaning Up

  • Shopping for Essential Items
  • Laundry
  • Eating or Feeding

Preventing Readmissions

READMISSIONS

Medicare has been measuring the readmission rates of hospitals for the purpose of picking out and penalizing the weakest performers since October 2011. Starting October 2012, the bottom 25% of hospitals lost 1% of their Medicare reimbursement (based on diagnosis-related groups). This penalty will increase to 3% by 2015. To reduce readmissions, most hospitals nationwide have increased their efforts, so hospitals who are current leaders in this measure cannot rest on past successes.

However, readmissions are not a hospital or physician function alone. Home health, hospice, rehab facilities, and even the patients themselves play a role in the factors that can decrease hospital readmissions. At Advanced Seniors Healthcare Group, Inc., we seek to prevent unnecessary readmissions through skilled care and patient/caregiver education.

We prevent problems before they begin through careful observation and education. Nurses visiting the home on a regular basis can detect worsening health conditions in time, so they can be managed without lengthy inpatient stays. We educate patients and caregivers on the symptoms and warning signs of their condition, involving them directly in care.

Providing additional direct care and teaching the patient to adhere to their medical plan often prevents the need for inpatient stays or ER visits. When patients are newly discharged from inpatient facilities, follow-up with their primary care doctor and adherence to discharge instructions are notoriously low. We enhance facility-to-community care transitions by providing proactive, in-home care and encouraging patients to follow up with their primary care physicians.
